Family Representative

A family representative is someone designated by you to release your medical information to others. The representative is responsible for the release of personal information regarding medical records, prescription medications, etc. Only one representative may be designated. Once you are admitted to Northland Cancer Center, please take the time to select someone doctors and family members may go to for this important information.
