Medical Records Request

If you wish to request copies of your medical record please visit the Health Information Management office located on the 2nd floor Main Hospital to sign a release. There will be a charge for copies of medical records requested for personal use. If you are physically unable to visit the hospital's Medical Records department, please send a written request for the release and we will mail one to your provided address.
