Home Health – 20 Years of Service
Aug. 20, 2014, marks the 20th anniversary of North Kansas City Hospital Home Health Services. Home Health helps referring physicians manage their patients at home and helps prevent unnecessary rehospitalizations. In addition to providing services to their patients in Clay, Platte, Jackson and Ray counties, case managers work with referring physicians to assist with Medicare reimbursement and patient reporting.

Continuum of Care
To ensure there is no lapse in patient services, Home Health performs patient intake, admissions and offers services every day, including weekends. Additionally, they have an on-call RN on nights and weekends to answer patient and family questions and make home visits as appropriate.
Services will be further enhanced with the addition of remote patient monitoring this fall. For patients receiving this specialized service, weight, blood pressure and oxygen saturation will be tracked through telemonitoring equipment and reported to their physicians.
