Information for Cancer Patients

Please bring to your first appointment:

• Photo ID (driver’s license, passport, military ID, or other government-issued documentation)
• Insurance card
• A list of your medications
• Past medical records related to your condition
• Lab results, test reports and imaging (CD) of any recent testing performed at another facility
• Name and contact information of your referring physician and primary care physician
• Questions for your doctor

Cancer treatments vary depending on your diagnosis and other factors. Response to treatment also varies from person to person. These guidelines will help you understand the treatment process and deal with many common side effects you may experience from your cancer or its treatment. Your own experience will be unique, of course, so we encourage you to ask questions at any time.
